So I got my FISH test results back, "atypical and should be interpreted with caution".  Trisomy for chromosome 3 but chromosome 7 and 9 were fine.   Spoke to my Dr and we are just going to stay the course.  I will have a third round of maintenance BCG in June and continue to get scopes along the way.  Best of luck to all.....

It happened to me a few years ago. My GP did a cytology that showed "atypical cells" so, of to my uro I went. He looked at the slide and concluded : "Why are you here", nothing to worry about. I deferred to my specialist and never looked back.

Joea73's response seems to corroborate this.

DX 5/6/2008 TAG3 papillary tumor .5 CM in size. 2 TURBS followed by 6 instillations of BCG weekly with a second round of 6 after a 6 week wait.

Thanks Joea73 for that information. Its always hard to listen to the right side of my brain as it says, "lets wait until the DR has all the information then move forward" while the the left side of my brain is running around shouting, "The sky is falling!!!"

BCG can often cause changes to cells of the lining of bladder.   According to a study, within 100 days after BCG treatment, sensitivity ( there is a tumor when cytology says positive) was 62% vs 82% when cytology was done after 100 days of BCG treatment, meaning that false positive rate increases by over 20% when cytology was done within 100 days after BCG treatment. 

In another study, translucent nuclei and prominent nucleoli, vacuolization of cytoplasm, and eosinophilic cytoplasmic inclusions were frequently observed in urothelial cells as well as an increase in granulocytes, especially within 3 months after BCG instillation therapy.  

It is possible that the cytopathologist was not told that you had BCG treatment, otherwise the report could be more specific rather than just several clusters of atypical urothelial cells.

Question for the group.  Has anyone every had a clear cystoscopy with no sign of a reoccurrence but the urine cytology comes back with "several clusters of atypical urothelial cells present"?  I am anxiously waiting on the results of a FISH study.  As a background, I was originally diagnosed in 2005 and had a low grade tumor removed.  in 2006 I had a reoccurrence and after tumor removal went through induction BCG.  All clear after that until 20022 when my annual scope found CIS. Two rounds of Induction BCG and then two rounds of Maintenance BCG.  Scoped this week and nothing see but due to this #$%*& MyChart app, the urine cytology results pop up on my phone immediately.  :)  My Doctors keeps telling me to wait until I chat with him, but humane nature being what it is, I search the Internet and go down to many rabbit holes. 

Anyway, I'm just curios if anyone has had a clean scope but positive results for the Urine cytology and FISH tests.
