One of the things we never think to ask the surgeon is what type of neobladder is he constructing? They all have their favorites..some work better than others.
The configuration of the right colonic and ileocolic neobladders resembled the shape of a rugby ball. The configuration of the sigmoid neobladder was oval. The right colonic and ileocolic neobladders tended to be located along the right side wall of the pelvis. The sigmoid neobladder was located in the center of the pelvis. Daytime and nocturnal continence was not affected by either the type of neobladder or its configuration or position. Neobladders located in the center of the pelvis exhibited a better maximum flow rate than those located along the right wall of the pelvis.

CONCLUSION: The technical difficulty in constructing the 3 types of neobladders was approximately the same. For better voiding a neobladder should be located in the center of the pelvis

I believe the only diversion to have a valve to prevent back up into the kidneys is the Indiana Pouch(IF the surgeon does it correctly)...now there might be some surgeons out there that have figured out how to do it differently but its certainly a question to ask.

I had an RC 2 and a half years ago. Renal failure in July of 09, due to scar tissue. I now have a nephy tube from my left kidney to my bag. I had a slight blockage in July of this year and I just went through another full blown blockage this past month with kidney infection. I had severe kidney pain, similar to labor pains. I went to my primary yesterday and he gave me Cipro to keep should I get another fever. When they changed my tube on the 29th of Sept, they flushed it first and it had sludge [puss I think] and crystals that were blocking it. Things are better right now, but who knows when it will happen again. Good luck.

I have had more than a few kidney infections, leaking especially at night and other difficulties since getting the neo over 3 years ago. Uroflow studies revealed that the top right portion of my neo has folded down which traps the flow from the ureter to the neo. This back pressure flows into the kidney causing the hydronephrosis. At night when I lay flat, the leaking goes nuts as the urine can flow to the neo more freely. It all promotes the kidney infections. The right side is also where I have 2 narrow ureters.
The left side has distention in the ureter due to scar tissue binding. I get painful spasms abdominally that are similar to the pain I had with a strangulating hernia 20 odd years ago.
Surgery is scheduled for Nov 2. They hope to remove scar tissue and anchors on the vaginal walls. (The anchors pull when I hold about 300 cc's) They will take out the urethra, and try to run my appendix thru my navel to make the resivior more like an indiana pouch. If they can't I will end up with a bag. I do wish I had the indiana from the start.
My other option was daily antibiotics. I have had cdiff 3 times sine the neo and am terrified of dealing with it again...antibiotics increase the risk.
The drs says my neo has failed. I am confident it is not from infrequent voiding as I have to void every 2-3 hours and it doesn't hold much before I feel pain. I do cath twice a day for residuals but I think the residuals are there because of what is trapped behind the fold.
I think the duplicated ureters make it easier for flow to get trapped in the kidney and I have no idea if an overfull neo can back up into the kidney.

It really is starting to seem like the more i learn about the
whole neo-bladder experience, the more i realize how much i don't know. As i progress through this latest problem and read posts from everyone, i realize i need to try to study and understand more on the subject. What do i need to do to protect my kidneys, what are the signs to watch for? I know
right now there are risks and problem that i am totally
unaware of, don't know what to watch for or what to ask. Is there already a well put together informative artical on living with a neo-bladder? I think like with any cancer, one
goes through a time of just trying to survive the cancer, then for me a time to relax a little and enjoy coming out on the other side. Then like now for me at least, how to live
with all the changes and what to watch for.
Example: In '91 when i had bone cancer i had a lot of chemo.
When i was told last year i had bladder cancer, i when to my
present oncologist to get his opinion. He took one look at my records and said " Oh yes there is a very high rate of bladder cancer for people that had that type of chemo". It would be very nice to know what to watch for this time.
Let me say one last thing on the subject, then i will shut
up. You only need worry about what to watch for in the
future, if you have a future. At this time i am lucky to
have one!

Thanks for opening this subject. I was at a conference this week and had a very painful stab in my kidney area. I was concerned that I was about to have a bad time right in the middle of someones presentation. I then realized that I had not visited the restroom in almost 5 hours!

This neobladder does not give off the same signs of fullness as a normal system. I was simply overfull!

My question is; does the neo bladder fill up and then overflow into the kidney and could this be a cause for concern? Did I possibly do damage to the kidney by waiting so long?

I am not going to let it go this long again.

Cynthia and Ann, Thanks for your responses. Just like i told
Mike, you all amaze me with all the help you provide. THANKS!
I hadn't really thought about the double ureter thing or the
back up into the kidneys, until it was brought up here. I guess i have been more focused on the WHAT IF. Being a veteran
of this cancer stuff you would think i would be over that, but
i guess not. After reading your responses and with the fact
my kidneys have been weakened from chemo anyway, i suppose i
better get more proactive on getting some answers. Will start
pressing my Dr. for a few more answers and trying to find out
how familiar they are with these problems.
