Thanks for the link. I printed out the pages. My doc already has me doing the first one, and I'll try some of the others. I got him to explain in detail what's going on with my plumbing, because it helps me to better do the exercises. I'm perplexed how my old bladder retained urine, but it obviously worked well. What a magnificent machine.

So is anyone having trouble with mucus? This is my original scare when they first explained the whole neo-bladder thing (you want me to do WHAT to myself?!) I'm still irrigating thru the SP tube for two weeks, and pulling a LOT of crud out, which says my urination is not up to par. Sorry for the details. Good incentive to practice, practice, practice.

Artificial urinary sphincter placement in men after cystectomy with orthotopic ileal neobladder: continence, complications, and quality of life

Abstract: Objectives. To review our experience regarding the overall efficacy and safety of an artificial urinary sphincter (AUS) in men with stress urinary incontinence (SUI) after cystoprostatectomy with an orthotopic ileal neobladder.Methods. We performed a retrospective review of 5 men who underwent placement of an AUS (AMS 800) for severe SUI after radical cystectomy with an orthotopic ileal neobladder. Incontinence symptoms and quality of life were quantified using two validated continence questionnaires (Urogenital Distress Inventory Short Form and Incontinence Impact Questionnaire Short Form) and a brief addendum questionnaire. The degree of continence, perioperative and postoperative complications and infections, symptom distress, quality of life, and patient satisfaction were compared before and after AUS placement.Results. Complete (0 pads per day) or social (1 pad or less per day) continence was reported in 5 of 5 patients after AUS placement. The average pad usage significantly decreased from 6.2 to 0.6 per day (P <0.001). No significant perioperative or postoperative complications or infections were noted. Symptom distress, quality of life, and patient satisfaction were significantly improved after AUS placement (P <0.01, <0.0001, and <0.001, respectively).Conclusions. With short-term follow-up and a limited number of patients, the placement of an AUS for treatment of SUI after bladder substitution is well tolerated and reliable and has a positive impact on patients’ quality of life
This can be done and probably none of you now remember Zachary who was treated at USC/Norris. He had the artificial sphincter placed one year after his radical and it did the trick...but i would only trust a super surgeon to do this.

This is one of those subjects that will not go away! The magic of modern medicine can do wonders. It saved us all, and yet they cannot seem to stop us from wetting the bed! The older we get, the more we go back to our childhood! We changed diapers for our kids. My wife stopped using pads. I am now using pads and diapers!

What a Life!

I have found (after 2 years out) that I still cannot go to bed totally confident that I will not turn on the fountain during the night. I cannot predict when this will happen, so I still wear a pad.
During the first few months I found that Super Max Diapers were the key during the day; And the Hoover Dam was the way to go at noght! :laugh: :laugh: :laugh: Nothing seemed to hold back the flood!

But it does get better. Keegles help in time. Going on a very Regular basis is the best way to gain some composure. A good schedule is very helpful.

I am here to say that "you will find continence".... someday... and somewhat. Even those of us 2 years out are still finding ourselves leaking during hard excercise and at night sometimes. I have even leaked during a sneeze or a good belly laugh. Embarrassing... Yes! The end of the world... No!

The best full on pads I have found are Depends for Men. They are just starting to push these in their advertising.

I guess if this is our only issue, then we are surely on the road to recovery and we have to have something to complain about. This is a sure sign of Moving on!

Red, Patients is the key. This is the most difficult time.
You should see improvement soon. I remember the day after I had my catheter removed. I thought what is the big deal. I stood up, and completetly voided all over myself, slippers, floor , etc.
I can look back and laugh now, but it wasn't very funny at the time. You will start getting the hang of things in a couple weeks.

More than you ever wanted to know about sphincters

OK. I looked it up. Didn't know all of that. It would make sense that the internal one would come out with the bladder since it the same muscle as the bladder.

You don't have to concentrate on keeping the voluntary sphincter closed. It just works out as you do your exercises to strengthen the other pelvic floor muscles.


Accroding to the wiki link above, women don't have the internal sphincter and in men it is there to prevent seminal reflux during ejaculation. So, it may not have anything to do with urination anyway.... And, since you no longer have a prostate, you will no longer have ejaculations so you don't need the internal sphincter.
