Spoke to Dr. Lane at The Cleveland Clinic this afternoon and have set up a meeting with their Stoma Department for 11:30 Wednesday. I hope to have all my questions answered tomorow concerning the Ileal Conduit vs the Indiana Pouch.

Have decided to have the surgery using the Cleveland Clinic robot. Dr. Lane said the blood loss will be less but the surgery slightly longer. With the robot, the incision will be much smaller and recovery should be quicker and easier. Let's hope so......

I have the option of having the RC and prostate removal sugery either on April 15th. or April 30th. (the only available dates for the robot). Dr. Lane said waiting until April 30th. should NOT effect the outcome of the surgery or the spread of the cancer to the nodes.

I will probably make the final decison on the surgery date AFTER I have come to grips with my decision on the choice for the urinary diversion.

Ok i see a lot of changes are going to come about at The Cleveland Clinic.
Previously, Dr. Klein was head of section of Urologic Oncology in the institute and was appointed February of this year as head of urology. Its obvious that he is bringing in his own people. Dr. Lane has had privledges at the Cleveland Clinic but not on staff. I would still ask him how many he does in a year.
Some cages are rattling for sure. Drs. Campbell and Stephenson were the go to's for bladder cancer. Maybe Dr. Gill is going to bring them out to USC/Norris?

I think each surgeon has their own way of doing things and drains or stents they place in you. I've read several accounts of an Indiana Pouch and what drains you will have and heck i didn't have all of those things.....nor did i have any stents. I had 2 Jtubes for drainage..the suprapubic tube...but no tube into my stoma at my navel at all...maybe because it was done laproscopically? Or because at the navel the skin does not think its a wound and try to heal over. Unfortunately i did have that nasty nasal gastric tube....some places don't use them at all...Indiana doesn't.

And when the suprapubic is no longer needed, they just pull it out. No need for further surgery. It just closes up on itself, and the hole heals. So don't base your decision on the number of tubes coming out when you come out after the bladder comes out (I would insert an emoticon here, but the function seems to be temporarily broken....imagine a smiley face)

Doug,

I believe that it's suprapubic. Since the Indiana pouch will eventually hold a good amount, it needs to be "trained". So when a patient is discharged from the hospital, this drain allows urine to flow from the pouch for several weeks while it heals. After a few weeks, the drain will be removed and the patient will be able to catherize the pouch to drain the urine. The interval between each catherization is gradually increased to "train" the pouch to hold larger amounts of urine.

Since there's no permanent connection to an external bag with an Indiana, the suprapubic just provides a temporary method to drain urine until things have healed.

For whatever reason Dr. Lane is NOT listed on the Cleveland Clinic website; however Dr. Eric Klein (head of dept. of urology) said he trained Dr. Lane and he has been on staff at CCF for the last six years. Dr. Klein highly recommends Dr. Lane. I sure hope he is right......

I hope to get more of my questions answered this week when we meet with both Dr. Lane and the Stoma Dept. I still have NOT made the decision as to whether to have the ileal conduit or the Indiana pouch.

Dr. lane said he has performed an RC with prostate removal in 3.5 hours with the ileal conduit. He said to add 60-90 minutes if I decide on the Indiana pouch. He also said with the Indiana pouch I would leave the hospital with a cath in the stoma and a subrepubic tube. What is a subrepubic tube?
