T4 Bladder Cancer

Hello SMIKEY, I hope and pray things are going well. I had stage 3 bladder cancer, with a high grade tumor. 12 weeks of chemo, 4 cycles pre op. I am now just 6 months with my neobladder. I had to self catheterize only to help remove mucus from the new bladder and irrigate with saline. Maybe I'm lucky or I just don't remember, but it wasn't all that bad. The down side is the incontinence, witch everyone says will improve. With that said I look forward to Spring and my new golf game. All the best to you and stay positive. Pete

SMIKEY,

Yes the j pouche or indiana pouche will work while doing everything you did before, my stoma is pretty small and the small amount of mucous seals it right up so I've had no leakage thus far. I can hold 700cc of urine before i have to empty it. A dull tummy ache is my signal to catherterize. So far if i watch my nitely water intake I can sleep from 11:00pm till 6am. Starting out is harder since you need to slowly stretch the pouche. Convience and short learning curve are why I chose the j pouche.

Absolutely true Sara, because of the possibility of constant catheterization I went with the Indiana pouche!

I have reviewed the Cleveland clinic info which was very informative and I am looking at what my surgen has been performing or what he is good at. Also I am trying to make a decision on my future life style that would minimize infections, leakage, or other complications, being that I only have 1 kidney. Thanks

I believe that the "j-pouch" is the "Indiana Pouch" described in the reference web page I suggested earlier
my.clevelandclinic.org/health/treatments/12546-urinary-reconstruction--diversion

While this diversion does require catheritization, it is through a specially prepared stoma in the abdomen, not what you usually think of when you imagine a catheter. There is no external bag.

The neobladder uses your natural plumbing. It requires a bit of "training" to get it to work. It is if it does NOT work
that it requires using catheters through the urethra.

Hope this helps
