Had second TURBT this morning for restating HG papillary carcinoma. No new tumors to report. Should have report in a week or two along with a plan.

8/9/16: 2cm tumor found on CT
8/10/16: cystoscopy
8/18/16: TURBT; HG Non-Invasive
9/29/16: 2nd TURBT clear
Completed first BCG regimen: 1/31/17

Don't beat yourself or your URO over the report too much. For whatever it is worth my 1st path report read very similar to yours including a papillary tumor (which he also saw in his 1st scope). Second TURBs usually done when the path report reads any high grade. While I and Sara Anne are not a doctor, it is simply part of the waiting game. I believe you are on to the way to beating this!

DX 5/6/2008 TAG3 papillary tumor .5 CM in size. 2 TURBS followed by 6 instillations of BCG weekly with a second round of 6 after a 6 week wait.

Your urologist is doing absolutely the right thing....wish more of them would do this. Doing a second TURB increases the accuracy of the diagnosis significantly. There are some question arising from the previous biopsy samples and, now that he knows where the problem areas are, he is going in for a second, and better exam. From the path report, it appears that there was not enough tissue in the samples to identify the under layers
"-ADEQUACY: MUSCULARIS PROPRIA NOT IDENTIFIED" and so they need to get some samples with this included to determine if the cancer has spread into the muscle.

During the first TURB, the urologist is sort of going in "blind"....he knows where the tumor is, but not really how extensive it is. The second time he will pay special attention to the areas that are questionable. When he gets the results back, he (and you) will have a MUCH better idea of what you are facing.

Know that you hate to wait and you hate to go through this again, but it is for the best.

Wishing you luck

Sara Anne

Talked to urologist and oncologist and still confused. This indicates apparently that it's not clear they got it all. Having a second TURBT on 9/29. All I know for sure is that it is high grade. I guess but the staging cannot be determined nor if/how invasive

Gross description:
A. BLADDER TUMORS: Received in formalin in a specimen container labeled with patient name and "bladder tumors" are multiple pieces of tan red mucosal tissue measuring from less than 0.1 to 1.4 cm in greatest dimension. The specimen is entirely submitted in cassettes A1 and A2.
Microscopic exam/diagnosis:
A. BLADDER TUMORS, EXCISIONS:
-TUMOR TYPE: INDETERMINATE
-HISTOLOGIC TYPE: UROTHELIAL CARCINOMA
-ASSOCIATED EPITHELIAL LESIONS: NONE IDENTIFIED
-HISTOLOGIC GRADE: HIGH GRADE
-TUMOR CONFIGURATION: PAPILLARY
-ADEQUACY: MUSCULARIS PROPRIA NOT IDENTIFIED -LYMPHOVASCULAR INVASION: NOT IDENTIFIED
-MICROSCOPIC EXTENSION: NO DEFINITE LAMINA PROPRIA INVASION
IDENTIFIED
